1. Medical Consent & Liability Waiver

I acknowledge that football is a physical contact sport and carries inherent risks of injury. I agree that: I participate voluntarily and at my own risk. The league, its organizers, coaches, referees, and staff shall not be held liable for any accident, injury, illness, or medical expenses incurred during training, matches, or related activities. In case of a medical emergency, I authorize the league to arrange immediate medical treatment if required. I have disclosed any relevant medical conditions or allergies.

2. Code of Conduct

I agree to: Show respect to referees, teammates, opponents, and staff at all times. Avoid abusive language, harassment, discrimination, or violent behavior. Play fairly and uphold the spirit of the game.

3. Equipment & Safety

I am responsible for wearing proper football gear (shin guards, boots, etc.). I will not use unsafe or unauthorized equipment. I accept responsibility for maintaining my own gear.

4. Damage & Liability

I will be held responsible for any damage I cause to venues, facilities, or league equipment. The league is not responsible for loss, theft, or damage of personal belongings.

5. Insurance Disclaimer

I understand that personal sports/medical insurance is my responsibility unless explicitly provided by the league. I release the organizers from liability for costs related to injuries or accidents.

6. Photo/Video Consent

I grant permission for the league to capture and use photos/videos of me during matches and events for promotional, social media, or documentation purposes.

7. Management Decisions

I acknowledge that any rules, scheduling, or organizational changes made by the league management are final. Such decisions cannot be challenged and I agree to abide by them fully.

Declaration

I have read and understood the above terms and conditions. By signing this form, I agree to participate in the Football League under the stated rules and policies.
